Passport Program Services

Passport Program Services

Passport Program provides individualized funding to people over the age of 18 with a confirmed developmental disability. This program is funded by the Ministry of Children, Community and Social Services (MCCSS). Passport funding provides a variety of support options to fit the needs and goals of each person receiving support. It can be used to access community support and participation, caregiver respite, or to hire a support worker.

Family Service Thames Valley can help families manage their Passport funding through coordination of supports, hiring and supervising support staff and managing the Passport funding according to the Ministry of Community and Social Services guidelines.

If you currently have Passport funding and are looking for coordination support, please contact Community Services Coordination Network (CSCN) at 1-877-480-2726. We receive referrals through CSCN.

Contact Developmental Services Ontario to apply for adult developmental services, including Passport. If you’re eligible, staff at the DSO will help you complete an application package to assess your needs and connect you with available services and support.
